Pain counters are a type of counter introduced in Tempest. They are used to deal damage to a creature but can also deal damage to the player using them as well.

Description

Pain counters first appeared on Torture Chamber in Tempest. It is an artifact that gains a Pain counter each upkeep. It then has an activated ability to remove all pain counters and deals damage to the target creature equal to the counters removed. If the counters are not removed during a turn and remain on it during the end step, the player takes damage equal to the number of counters.
