The Meditation Plane Breach was a 4564 AR confrontation on the Meditation Plane during the Omenpath Era that resulted in the escape of Nicol Bolas from his imprisonment. The incident occurred when Jace Beleren attempted to use the plane's power to alter history, inadvertently destabilizing it and forcing all present to flee. In the chaos, Bolas escaped into the Multiverse, and Ugin returned to Tarkir to end the multiversal dragonstorm outbreak.

Description

In the years after the New Phyrexian Invasion, Omenpaths began opening across the Multiverse. One such Omenpath connected Tarkir to the Meditation Plane, and magic leaking through it from Ugin's presence triggered an outbreak of dragonstorms across many planes.Seeking the cause, Elspeth Tirel traveled to Tarkir and joined Narset in investigating. After hearing a mysterious voice urging them to find "the temple in the heart of storms," they crossed the Stormplains and located an ancient temple containing the Omenpath.

Within the temple, Elspeth and Narset encountered Jace Beleren and the creature Loot, who had entered the Meditation Plane with Vraska by following Loot's innate knowledge of the Omenpaths. At first, the Realm appeared differently to each visitor, shaped by their thoughts, until Narset cleared her mind and revealed its true silvery form. There, the group found Ugin and the nameless dragon who had been Nicol Bolas, both bound in an eternal lockdown. Ugin rebuked Jace for breaking their agreement to keep the plane secret and warned them to leave. Elspeth's presence prompted Bolas to seek information, and in their brief interaction, she inadvertently revealed his name to him.

While the others were distracted, Jace stole Ugin's Spirit-Gem and began casting a complex spell that combined the Gem's power, Alquist Proft's illusion-manifestation technique, and the Realm's reality-altering properties. His goal was to reach back through time to undo the Phyrexian Invasion and restore lost futures. Elspeth attempted to stop him, believing he might be working with Bolas, while Vraska also tried to intervene. The spell briefly appeared successful, reshaping the Realm into visions of alternate realities, but Jace lacked the endurance to maintain it. The construct collapsed, fracturing the horizon into mirrored shards and unleashing a wave of void that erased everything in its path — including Jace, who shattered like glass.

The catastrophe forced all present — including Bolas — to flee the collapsing plane. In the confusion, Bolas overpowered Ugin and escaped into the Multiverse. Ugin emerged on Tarkir, where Sarkhan Vol and Taigam had performed a corrupted Stormnexus Ritual to control the plane's wild dragons. Ugin swiftly defeated Sarkhan with ghostfire, ending the dragonstorm outbreak but leaving many dead among the Abzan forces led by Ajani Goldmane and Felothar. In the aftermath, the Meditation Plane began to reform, and a reflected silhouette of a blue-cloaked mage appeared in its waters — a sign that Jace had reformed as the plane's custodian.
