Inserts a compact, horizontally-oriented alphabet-based table of contents instead of the usual auto-generated one.

Parameters:

• top=no to prevent the "top" label from showing (it's not necessary if template is near the top already)
• plusminus=yes to show ± link - use that character as section name or span id
• refs=yes to show "References"
• exlinks=yes to show "External links"

Requires literally "A", "B", "C", etc sections on the page it's used on, OR "an empty placeholder span (e.g. <span id="Q"></span>)", to link to that section of the page.
